I think this is to document that you asked the cop to honor your 4th and 5th Amendment rights and help get any charges thrown out should the cop violate your 4th or 5th Amendment rights. You also have a constitutional right to a lawyer. If you say, "I want a lawyer," the police are supposed to stop questioning you. Sometimes, though, they don't hear you demand a lawyer.
